At Canonsburg Mutual we value you as a customer
and protecting the privacy of information about you is a responsibility
we take very seriously. In order to serve you better, having
accurate information about you and your properly permits us
to provide appropriate insurance products, better customer
service, increase the efficiency of our operations and to
comply with legal and regulatory requirements. We understand
that this nonpublic personal information needs to remain confidential
and therefore have this established policy to protect that
information.
When you apply for any type of insurance, whether
it's auto or property, you disclose a certain amount of information
about yourself to us. The collection, use and disclosure of
such information are regulated by law, In addition, Canonsburg
Mutual Fire Insurance Company and its agents recognize the
confidentiality expectations of our applicants and policyholders.
Therefore, it is the policy of Canonsburg Mutual and its independent
agents to:
- Collect only information necessary or relevant to our business.
- Make a reasonable effort to ensure that information we act upon is accurate, relevant, timely and complete.
- Use only legitimate means to collect information.
- Make personal information available externally only to
respond to legitimate needs, to regulatory or other government
authorities or as otherwise permitted by law.
- Limit employees' access to those who need to and are trained in the proper handling of personal information.
INFORMATION COLLECTED:
The majority of the nonpublic personal financial information
we collect about you comes from:
- Your application for insurance, renewal application
and other forms you complete to provide information for
that application process, such as name, address, phone number,
Social Security number, personal property inventories and
photos of your property.
- Sometimes we may contact you by phone or mail to obtain additional information.
- Depending on the nature of your business transaction, we may need additional information about you or other individuals proposed for coverage from outside sources, such as motor vehicles records, loss information reports, credit reports, court or other public records, and inspection bureaus to verify value and conditions. (We will comply with state law and the Federal Fair Credit Reporting Act)
- Other insurance companies.
- Once insured by us, we may also collect information about
claims you file under the policy, claims asserted against
you under the policy or claims you make against an insured
under Canonsburg Mutual insurance policy.
WHAT IS DONE WITH THIS INFORMATION:
Information which has been collected about you,
which we keep, will be contained in either our policy records
or in your agent's files. We review it in evaluating your
request for insurance coverage, in determining your rates,
issuing and servicing insurance policies, and setting of claims.
Your agent may use information about you in his/her files
for Insurance marketing. purposes or to help you with your
overall insurance program. Should you cease to be one of our
policyholders or after your claim is settled, we will continue
to follow the privacy policies and practices as described
in this notice.
TO WHOM WE DISCLOSE THIS INFORMATION:
We do not disclose any nonpublic personal
information about you to anyone, except as is permitted by
law, We may be permitted by law to share information about
you without prior permission under certain circumstances to
certain persons and organizations such as:
- Our affiliated insurance companies.

- Parties who perform a business, professional or insurance function for our company, including our reinsurance companies.
- Independent claims adjusters. appraisers, investigators
and attorneys who need the information to investigate, defend

reporting agencies as reasonably necessary in connection
with any application, policy or claim involving you.
- Insurance support organizations which are established

verify coverage or conduct as an audit of services.
- Insurance regulatory agencies in connection
with the regulation of our business.
- Law enforcement or other government authorities to protect
our legal interest or in cases of suspected fraud of illegal
activities.
- Authorized persons as ordered by a subpoena,
warrant or other court order or as required by law. Certificateholders
or policyholders for the purpose of providing information
regarding the status of all insurance transactions.
- Leinholders, mortgages, lessors or other
persons shown on our records as having a legal or beneficial
interest in your policy.
ACCURACY OF YOUR INFORMATION AND QUESTIONS:
Our goal is to keep your information accurate,
current and confidential. You have the right to know what
kind of information we keep in our files about you, to have
reasonable access to it and receive a copy. Write to us if
you have questions about information you would like to receive.
Provide your complete name, address, type of policy and policy
number that was issued or applied for with us. Certain types
of information generally collected when evaluating claims
or possible lawsuits need not be disclosed to you. We will
respond to you within a reasonable period of time with the
amended or corrected information or the reason why we believe

Canonsburg Mutual reserves the right to modify
or remove parts of this privacy statement at any time. Notice
will be provided to you in advance of any changes that would
